Referral process for Psychiatrists/Paediatricians

Referring with an Eating Disorder Treatment & Management Plan (EDP)

Complete the top of the EDP as follows:

We accept all new eating disorder referrals (for both in-person and Telehealth).

Please ensure that the patient has signed the consent box at the end of the plan before submitting. If you are the care co-ordinator for the patients eating disorder treatment, please provide details on other health professionals working with the patient if possible to maximise continuity of care processes.

Once the plan has commenced we will write to you after the first appointment and at the end of the 20 sessions. We will also contact you if we have any concerns or need to share or acquire any other relevant information between sessions 1 and 20.
